Manual lymphatic drainage (MLD) is one of the most misunderstood therapies we offer at Lambert Sports Clinic. Many people have heard of it in the context of post-operative recovery — but it has a much broader range of applications, from managing chronic swelling to supporting immune health and even athletic recovery.

This guide covers everything you need to know: how the lymphatic system works, what MLD actually involves, who benefits from it, and what to expect from treatment.

What Does the Lymphatic System Do?

The lymphatic system is a network of vessels, nodes and organs that runs throughout the body, parallel to the blood circulatory system. Its three main functions are:

  • Fluid balance: Approximately 2–3 litres of fluid leak from blood capillaries into the surrounding tissues every day. The lymphatic system collects this fluid (called lymph) and returns it to the bloodstream, preventing swelling.
  • Immune defence: Lymph nodes filter bacteria, viruses, cellular debris and cancer cells from lymph fluid. The nodes produce lymphocytes — the white blood cells responsible for adaptive immune responses.
  • Fat absorption: Specialised lymphatic vessels in the small intestine (called lacteals) absorb dietary fats and fat-soluble vitamins from digestion and transport them into the bloodstream.

Unlike the cardiovascular system, the lymphatic system has no pump. It relies on skeletal muscle contraction, breathing, and tiny smooth muscle contractions within the vessel walls to move fluid. This is why inactivity — or disruption of the vessels through surgery — causes fluid to accumulate.

How Does Manual Lymphatic Drainage Work?

MLD uses very light, rhythmic strokes applied to the skin surface to stretch and stimulate the superficial lymphatic vessels, encouraging them to contract more frequently and move lymph fluid towards the nearest functioning lymph nodes. The pressure used is significantly lighter than any other form of massage — typically described as the weight of a 10p coin on the skin.

MLD was developed in the 1930s by Danish physiotherapist Emil Vodder. It uses a specific sequence of movements that follows the anatomy of the lymphatic system, beginning at the lymph node clusters (neck, armpits, groin) to clear capacity, then working distally to move fluid towards them.

Critically, MLD is not the same as regular massage. Applying deep pressure to lymphoedematous or post-operative tissues can actually compress and damage the delicate lymphatic vessels. A trained therapist will always use the correct, very light technique.

Who May Benefit from MLD?

Post-Operative Recovery

After liposuction, abdominoplasty (tummy tuck), BBL, breast surgery, or any procedure that disrupts lymphatic vessels, MLD helps clear swelling, reduce bruising and prevent fibrosis.

Lymphoedema Management

Primary or secondary lymphoedema (often following cancer treatment involving lymph node removal) is the primary medical indication for MLD, combined with compression bandaging.

Sports Recovery

MLD can help reduce delayed onset muscle soreness (DOMS) and tissue swelling following intense exercise, accelerating recovery between training sessions.

Immune Support

By stimulating lymph node activity and lymph flow, MLD may support immune function — particularly relevant during periods of illness recovery or high physiological stress.

Orthopaedic Injuries

Ankle sprains, knee injuries, and fractures can all cause significant local swelling. MLD can help reduce oedema and support healing in the early stages after injury.

General Wellness

Many clients find MLD deeply relaxing and report improved energy and reduced feelings of puffiness or heaviness, particularly in the legs after periods of prolonged sitting or travel.

Medical vs Cosmetic Recovery: Key Differences

Medical MLD (Lymphoedema, Post-Cancer Treatment)

Medical MLD is a highly specialised intervention typically provided as part of Complex Decongestive Therapy (CDT). It is usually combined with multilayer compression bandaging, skin care and specific exercises. Therapists providing medical MLD should have specific lymphoedema training. Treatment is often intensive (daily in the initial phase) and then ongoing for maintenance.

Post-Operative MLD (Cosmetic Surgery Recovery)

Post-operative MLD following cosmetic procedures is the most common reason clients visit Lambert Sports Clinic. The goal is to reduce post-surgical swelling, prevent fibrosis, ease bruising, and support smooth skin retraction as healing progresses. Sessions typically start within 24–72 hours of surgery (with surgeon clearance) and are performed 2–3 times per week initially, tapering over 6–8 weeks.

What Happens During a Session?

A typical MLD session at Lambert Sports Clinic:

  1. Consultation (first visit only): Review of medical history, surgical notes if applicable, current medications, and treatment goals.
  2. Assessment: Your therapist will observe and feel the tissue — checking swelling levels, temperature, skin condition and any areas of hardness.
  3. Treatment (30–50 minutes): Light, rhythmic strokes following the lymphatic pathway. You will remain clothed or with appropriate draping; the pressure will feel very gentle. Most clients find the treatment deeply relaxing.
  4. Aftercare advice: Hydration guidance, compression garment fitting advice (if applicable), and recommendations for your next session.
